Consent & Media Release Agreement

Effective Date: February 27, 2026

By submitting content, requesting care, or providing updates to Currents of Care (the “Platform”), you acknowledge and agree to the following:

1. Voluntary Participation

I understand that my participation with Currents of Care is entirely voluntary.

I am choosing to submit information, images, video, audio, written statements, or updates of my own free will.

I understand that I am not required to participate and may decline to provide any information at any time prior to submission.

2. Authorization to Use Name, Likeness & Story

I grant Currents of Care and its operators a worldwide, royalty-free, perpetual, irrevocable license to use:

• My name (or approved display name)

• My likeness

• Photographs or video I submit

• My written or spoken statements

• Details regarding my situation or care request

This content may be used in whole or in part, edited for clarity or length, and distributed across:

• The Currents of Care website

• Social media platforms

• Video platforms

• Promotional materials

• Future compilations or educational content

I understand that content may remain publicly accessible indefinitely.

3. Health Information Disclosure

If I share information relating to my physical or mental health:

• I acknowledge that I am voluntarily disclosing personal information.

• I understand that Currents of Care is not a healthcare provider and is not subject to HIPAA or similar medical privacy laws.

• I understand that once information is published publicly, complete removal from third-party platforms may not be possible.

I accept responsibility for any information I choose to disclose.

4. No Medical Advice

I understand that Currents of Care does not provide medical, psychological, or professional advice

Participation does not establish any provider-client relationship.

Any medical decisions I make remain my sole responsibility.

5. No Guarantee of Outcomes

I understand that Currents of Care makes no claims, promises, or guarantees regarding healing, improvement, or specific results.

I acknowledge that any outcomes shared by participants are individual experiences and do not represent guaranteed results.

6. No Compensation

I understand that I will not receive financial compensation for:

• Submitting a request for care

• Providing updates

• Allowing use of my likeness or story

Participation is voluntary and without expectation of payment.

7. Right to Edit or Decline

Currents of Care reserves the right to:

• Edit submissions for length, clarity, or tone

• Decline to publish any submission

• Remove content at its discretion

I understand that submission does not guarantee publication.

8. Release of Liability

To the fullest extent permitted by law, I release and hold harmless Currents of Care, its operators, and affiliates from any and all claims arising from:

• My participation

• The use or publication of my submitted content

• Any decisions I make related to my health or well-being

9. Consent for Minors

If submitting on behalf of a minor:

I affirm that I am the legal parent or guardian of the minor and have full legal authority to grant consent for publication and participation.

10. Revocation of Consent

I understand that I may request removal of my content in writing.

However, I acknowledge that:

• Removal from the original platform does not guarantee removal from third-party shares, reposts, or archived copies.

• Previously distributed materials may not be fully retractable.

11. Electronic Agreement

By checking the required consent box and submitting content, I acknowledge that:

• I have read and understand this Consent and Media Release Agreement.

• I voluntarily agree to its terms.

• My electronic submission constitutes my legally binding signature.
